Information we collect
When you join the housing interest list, we collect the contact, housing, household size, proposed occupant ages, independent-living, income, and optional contextual information you choose to provide. We do not ask for Social Security, bank, payment-card, Medicare, or benefit account numbers.
How information is used
Application information is used only to review housing inquiries, communicate with applicants, and determine whether to continue a placement conversation. Applicants are not subscribed to promotional marketing through this form.
Placement requests
When you submit a placement request through Tailored Care Placement Agency, a d/b/a of Tailored Care Living LLC, we collect the contact details of the person making the request and the information provided about the person needing placement, such as name, date of birth, contact information, housing needs, income range, daily-living needs, and background questions. Only submit information about another person that you are authorized to share.
Delivery and retention
Housing interest list submissions and placement requests are sent over an encrypted connection, recorded in a private spreadsheet, and delivered to our team by email. Only Tailored Care Living staff have access to these records. We keep them only as long as needed to review the request, follow up, and meet our business obligations. Our email and spreadsheet service providers necessarily process submissions to store and deliver them.
Security
We use server-side validation, input sanitization, spam controls, and encrypted HTTPS connections in production. No internet transmission can be guaranteed completely secure.
Your choices
You may choose not to submit the application. Please do not include sensitive identifiers or unnecessary medical details in any free-text field.
Changes
This policy may be updated as the website and business practices develop. The effective date will be revised when material changes are published.
